*Although we are listed as participating providers, our panel has been closed to new patients since 1997.  We accept this insurance for existing patients only, meaning those families that have continuously had this insurance before our panel closed. The only new patients that can be added to this panel are siblings or newborns within these families.  Once patients leave this plan they will not be accepted back again should their insurance revert to Optimum Choice/MDIPA.   Existing patients in our practice who have never had but then switch to Optimum Choice/MDIPA are considered new patients; we are therefore unable to add them to our panel.​

  • No United Family 1st (Americhoice)*
